Surgical Sperm Retrieval Procedure

What is Surgical Sperm Retrieval (SSR)?

When the couple opts for IVF treatment, the first step before the cycle, is for the male partners to provide a semen sample for a detailed analysis. In rare cases, sperm extraction process is necessary before the egg retrieval. It so happens that some men suffer from extremely low sperm count, also known as azoospermia. And, in some cases zero sperm in their semen. Therefore, with the help of Sperm Retrieval, the sperm can be collected directly from the testicles and is the best solution for male infertility.

Today, there are several medical procedures that help with the sperm extraction. However, the right method only depends on why the sperm isn’t present in the semen. Most commonly, the reason for zero sperm count is because of some blockage in the tubes that transport the sperm from the testicles. The blockage is usually due to conditions, such as testicular cancer, injury, etc. But, if there is no sperm, despite no obstruction, then the sperm production is faulty. In such cases, one will have to undergo an even more intensive treatment, such as TESE or Micro-TESE.

Types of Surgical Sperm Retrieval

PESA

Known as Percutaneous Epididymal Sperm Aspiration, it is a simple procedure. This method requires the least invasion. Here, the sperm is collected via a thin needle from the epididymis, which is a tube connected to the testicle. This procedure is performed under anesthesia. When PESA fails, NAB or needle aspiration biopsy is performed.

TESE

TESE or Testicular Sperm Extraction is performed only when PESA fails. The biopsy is extracted from a larger area of one’s testicular tissue. It is also a very popular SSR method.

Micro-dissection TESSE

This is usually preferred when the sperm count in the semen in zero. In this process, a micro-dissecting microscope is used to point towards the exact area to make sure the right tissue is removed.

TESA

Testicular Sperm aspiration is a method where the needle is used to extract the sperm using gentle suction. Tesa ivf is performed with the help of a biopsy gun. The sperm that is found is then frozen for future use.

Percutaneous biopsy

Almost similar to TESA, however, a different type of needle is used.

MESA

Microsurgical epididymal sperm aspiration is a method which is performed when PESA fails. During this procedure, anesthesia is used because a microscope is used. Also, a surgery might also be performed to rectify the blockage.
